Schedule: Monday through Friday 8AM-4PM OR 9AM-5PM
No weekends
Requirements:
- New York State LPN License
- BLS Certification
- Experience administering vaccinations on pediatric and adult patients is required
- Bilingual: English/Spanish is required
Responsibilities
- Review medical record for vaccination administration after receiving order from provider
- Obtain correct vaccinations from refrigerator, compare and confirm vaccination and dose ordered to what was retrieved
- Correctly identify patient in exam room using name and date of birth
- Administer vaccine as ordered
- Dispose of medical waste and hazardous waste as per OHSA guidelines and clinic protocols
- Document vaccine type, manufacturer, lot number, and anatomical location of administration in EHR
- Provide patient and/or guardian with the most current Vaccination Information Sheet (VIS)
- Update new VIS with notified by CDC and share with satellite sites
- Review Citywide Immunization Registry (CIR), generate list of needed vaccines and provide completed list to front desk / registration staff for recalls
- Update and review CIR of any patients; information and scan vaccine records into eClinicalWorks
- Monitor and record twice daily refrigerator and freezer temperatures, as well as minimum and maximum refrigerator and freezer temperatures, resetting the memory each time
- Assist Medical Assistants at satellite sites with any questions / concerns as well as performs satellite site visits for vaccine / refrigerator compliance when assigned
- Prepare Excursion Incident reports in conjunction with all satellite site MAs or other LPNs, calling manufacturers and sending completed reports to Director HIM & QI for review, sending review reports to DOH
- Prepare a Clinical Inventory Sheet (CIS) of the monthly inventory of all vaccinations at assigned site
- Stock medication cupboard, check medication cupboard monthly for any expired medications, rotating oldest medications to front, discarding those already expired
- Report VFC expired or spoiled vaccines electronically in the CIR for each specific site, and coordinates packaging of spoiled vaccines for pickup by the delivering entity
- Enter newly received vaccine information into eClinicalWorks (i.e., vaccine name, manufacturer, lot number, expiration date, quantity)
- Follow and demonstrate strict infection control guidelines to ensure proper clinic environment
- Follow sharps policy and procedure and demonstrate understanding
- Participates with clinical staff for in-service training, to ensure that nursing staff functions meet quality improvement standards
- Implement recommendations from Quality Improvement Team and Infection Control Committee, as requested
