Hiring for a MSW Social Worker for a healthcare facility in Boston, MA
Description: 30 hours/week – 8:30AM-3PM (Monday thru Friday).
Pay: $53/hr
Required Exp: 2 years + post MSW, Requires level of knowledge of Social Work process and community resources and agencies in relation to health, illness, and disability, generally acquired through one to two years on-the-job training.
Required Certs: MA Licensed Certified Social Worker or Licensed Independent Clinical Social Worker, CPR certification (AHA BLS healthcare provider).
Preferred Exp: Familiarity in working with patients in a community based environment. Experience in working with diverse patient populations, is also a plus. Experience coordinating care with case managers and care teams from external agencies (DMH, DDS, ASAPs) as well as community resources (transportation, Meals on Wheels, housing support) a plus. Experience with EPIC registration &/or scheduling is a plus.
Skill Level: Must be familiar with range of disabilities and physical, cognitive and mental health conditions that are common in the geriatric and with persons with disabilities. Must be comfortable assisting clients with ambulation and transfer techniques, assist and or lead small and large group programs, be a good communicator, comfortable working with other members of the health care team (i.e. nursing, MD, Case management), team player, strong time management skills, be self-directed, flexible and able to multitask.
Department Details: Program provides for adults who do not require 24-hour institutional care, but because of physical, cognitive and/or psychosocial impairment, are not completely able to live independently. Clients range from adults with disabilities (age 18 and over) through the frail elderly. This environment offers structure to clients who may be isolated, confused, or depressed. The program provides respite to the primary caregiver to be employed outside the home while their family member attends the day program.
Patient Ratio: Therapist covers all clients served at the ADH, with responsibilities divided between new client referrals, case management, client and family support and 1-2 hours/ day of direct client interaction.
Therapists manages the full ADH caseload; client documentation required within 2 weeks of admission and every 90 days thereafter.
