Substance Abuse Counselor

Location: Brooklyn NY (No parking available)
Setting: Outpatient – Substance Use – Harm Reduction – Opioid Treatment
Schedule: Monday through Friday 7am-3pm
Pay: $32-36/hour depending on experience
Important Notes:
- Caseload: 35-40 participants and 3 groups per week.
- 1 on 1 sessions (30-45 mins each)
- EHR: Smart, transitioning to TenEleven soon
Responsibilities:
- Conduct intake interviews, biopsychosocial, mental health screenings.
- Formulate treatment plans and treatment plan reviews, act as participant’s case manager to coordinate care.
- Perform individual counseling duties which include holding weekly individual sessions with clients, providing referrals to community-based resources (healthcare, social services, and administrative e.g. help obtaining insurance), and developing client schedules.
- Initiate and maintain client records in accordance with policies and procedures.
- Ensure accurate and timely data entry of all client information into the agency’s database; utilize electronic health record system.
- Facilitate assigned treatment groups.
- Perform drug testing as needed.
- Prepare monthly updates for court or other supervising agencies.
- Participate in treatment team case reviews.
- Consult with other professionals in support of client’s treatment.
- Complete discharge documentation as needed.
Requirements:
- MUST be bilingual Spanish speaking
- LMSW or CASAC required (will NOT consider a CASAC-T)
- Minimum of two years’ clinical/case management experience in working with clients that have substance use disorders required.
- Ideally looking for someone who has previously worked with a high caseload and is used to a fast-paced environment.
