Assignment Length: 3 Month Temporary Assignment
Location: Manchester, NH
Schedule: Monday to Friday – 8:00am-5:00pm
Pay: $55 – $60/hr, depending on experience
JOB SUMMARY:
The Perinatal Care Coordinator will coordinate all perinatal activities within the organization in collaboration with the Certified Nurse Midwife and Prenatal team. In addition, the role will assist with clinical tracking of CDF reports and all clinical indicators for grant reporting.
RESPONSIBILITIES & AUTHORITIES:
- Collaborates with the perinatal team to ensure that comprehensive, high quality prenatal care is delivered to all pregnant clients- including, but not limited to:
- scheduling appointments, pregnancy tests, prenatal intakes, triaging, case management, NSTs, injections.
- Develop linkages with medical and community agencies to receive and provide client referrals. Maximizes these relationships with referral agencies.
- Assess and triage patient’s need for treatment or advice via telephone call or personal encounter (walk-in).
- Determine, based on patient need, if patient needs to be seen by a provider and how quickly.
- Consults with provider whenever necessary in order to advise patient of most appropriate action.
- Maintains and manages nurse visit schedule including but not limited to program intake, Non-Stress testing, Rhogam and immunization administration. B/P follow up.
- Respond to patient calls in a timely manner.
- Maintain accurate, timely documentation in the client’s medical record of client contacts, visits, case planning, and client’s plan of care.
- Assist in the implementation of a MIS tracking system for CDF’s, OB referrals and case management of all prenatal clients. Create a recall system via mailings and telephone reminders.
- Assist in the maintenance of prenatal health education materials.
- Provide clear written and oral communication to both clients and employees.
- Attend and participate in other internal Health meetings as directed.
- If absent, responsible for reading of minutes and signing the documentation.
- Attend mandatory safety-related and infection control in-services as designated by Senior Management.
- Maintain licensure for RN status by ongoing professional education; maintain professional development.
- Sensitivity to cultural diversity of population being served.
- Maintain client age-related competence.
EDUCATION:
- ASN Required/BSN Preferred.
- Current NH RN license.
- BLS Certification Required
EXPERIENCE:
- At least 5 years’ Nursing experience.
- 2 year’ to include inpatient L&D or Outpatient Obstetrics.
- Experience in community health and women’s health, preferred.
- English language skills required. Bilingual (Spanish) Preferred.
KNOWLEDGE/SKILLS:
- Physical requirements include ability to extend hand(s) and arm(s) in any direction;

- Knowledge of data base and spreadsheet software systems.
- Computer skills preferred and basic financial skills, as well as analytical skills for trend and situational analysis desired.
- Ability to transport self to meetings, events and affiliated vendors.
- Ability to delegate tasks to other team members. Health assessment skills and coordination and delegation skills necessary.
- Intermediate electronic fetal monitoring certified or the ability to obtain within one year required.
