Location: In-Person Clinic, NYC
Setting: Primarily clinic-based with occasional home care visits (approximately once per month)
Schedule: Monday – Friday | 8:00 AM – 5:00 PM
Weekly Hours: 40
Salary Range: $210,000 – $240,000
Language Requirement: Bilingual Chinese (Mandarin or Cantonese) Required

JOB PURPOSE:
Under the direction of the Chief Medical Officer, the Site Physician is responsible for the professional, organizational, and administrative aspects of providing high-quality medical care to patients at CLHC/Day Treatment Center. This role requires fluency in Chinese (Mandarin or Cantonese) to effectively communicate with a predominantly Chinese-speaking patient population and to support culturally competent care delivery.


JOB RESPONSIBILITIES:

  • Direct the medical care and treatment of patients and oversee the clinical contributions of the interdisciplinary care team.

  • Collaborate with site nurse practitioners and other primary care providers on shared patient populations.

  • Identify opportunities to improve the quality, cost, and utilization of care for site participants.

  • Maintain a patient caseload as the primary care physician and deliver direct care within the Diagnostic and Treatment Center (DTC).

  • Lead the medical process during start-of-care events and conduct initial and ongoing assessments.

  • Participate in multidisciplinary meetings including case conferences, hospitalization reviews, and family/team meetings.

  • Develop and oversee the utilization review of medical services provided at CLHC/DTC.

  • Supervise ambulatory medical services and liaise with medical institutions, subspecialists, and community physicians.

  • Conduct staff medical education, supervise trainees, and offer in-service training.

  • Provide care in a variety of settings including the DTC and patients’ homes.

  • Serve as a clinical resource for staff in the DTC, including NPs, RNs, and MAs.

  • Develop and support preventive health and immunization programs.

  • Practice within the scope of licensure and established competencies.

  • Perform other duties as assigned.


QUALIFICATIONS:

Education:

  • Graduate of an accredited medical school.

  • Board Certification in Internal Medicine or Family Practice.

  • Board Certification or clinical experience in Geriatric Medicine preferred.

Experience:

  • Completion of residency in Internal Medicine or Family Practice is required.

  • Fellowship, diploma, or significant experience in geriatrics or palliative care preferred.

  • Minimum of one (1) year of experience working with a frail or elderly population (or appropriate training upon hire if under 1 year).

  • No malpractice history or license restrictions.

  • Excellent verbal and written communication skills.

  • Fluency in Chinese (Mandarin or Cantonese) is required to effectively support and communicate with a predominantly Chinese-speaking patient population.

Licensure:

  • Current, unrestricted license to practice medicine in New York State.

  • Valid Medicaid and Medicare provider IDs in good standing.

Additional Requirements:

  • Authorized to practice in the State of NY.

  • Medically cleared for communicable diseases and all immunizations up to date before direct patient contact.
