Locations: Brooklyn, Queens, Manhattan, Long Island (1 opening each)
Setting: Field (Home Care) | Schedule: Mon–Fri, 8:30 AM – 5:30 PM | Hours: 40/week
Salary: $135,000–$140,000 + $10,000 Signing Bonus!
About the Role
We’re looking for experienced Registered Nurses to lead clinical home care services across NYC and Long Island. As an RN Clinical Manager, you’ll supervise and support nurses and aides delivering top-tier care to patients in their homes. You’ll be the clinical backbone of our field teams, ensuring quality, compliance, and compassionate service—tailored to each participant’s unique needs.
This role is hands-on, management-heavy, and requires a leader who thrives in dynamic, community-based settings.
What You’ll Do

Oversee day-to-day clinical operations and supervise nursing staff in collaboration with Clinical Managers

Conduct joint home visits and competency assessments with nursing staff

Ensure care plans are implemented effectively and compliant with all regulations

Act as a clinical resource and mentor for Nurses, PCAs, and HHAs

Assist with new employee orientation and ongoing training efforts

Collaborate with care teams to develop personalized care plans that respect cultural and individual diversity

Maintain timely and accurate documentation per state and agency standards

Participate as an interdisciplinary team member, especially in PACE-related care

Conduct routine chart audits and report on patient status and care delivery

Navigate travel to patient homes in all weather conditions—reliable transportation required
What We’re Looking For

Education: RN with current, unrestricted New York State license. BSN preferred or equivalent experience (5+ years in healthcare)

Experience: Minimum 1 year home care experience (Certified Home Health Agency, LTCHP, or Licensed Agency)

Experience with frail/elderly populations required (or willingness to train)

PACE or MLTC program experience a strong plus

Solid organizational, presentation, and computer skills

Valid NYS Driver’s License and reliable transportation preferred

Knowledge of NY State Department of Health regulations
