Licensed Clinical Social Worker LCSW

Salary: Up to $70,000 

Schedule: Monday through Friday 8:00AM – 5:00PM
No weekends, no on-call

Requirements: 

  • 2+ years of experience
  • Master’s Degree in Social Work required
  • Current Florida State LCSW Licensure required
  • Experience working with substance abuse patients highly preferred
  • Experience working in community health preferred
  • EMR experience preferred

Responsibilities:

  • Provide supportive counseling, crisis intervention, and medical social service in a one-to-one and/or group setting
  • Identify patient needs and risks through a comprehensive psychosocial assessment
  • Develop and implement treatment plans along with patients and family members that address patient’s medical, mental, and health care needs, as appropriate
  • Elicit and interpret information from patients, families, community health service providers, and other sources
  • Identify problems, evaluate information, and draw conclusions when developing and implementing a treatment plan
  • Initiate and collaborate with interdisciplinary team
  • Exhibit excellent working relations with patients, visitors, and staff, effectively communicating the organization’s mission
  • Deliver services both virtually and in-person
  • Work effectively with inter-professional team
  • Establish therapeutic relationships with patients
  • Demonstrate knowledge on principles related to health promotion and holistic care
  • Demonstrate knowledge of SBIRT
  • Conduct biopsychosocials, treatment plans, addendums, and progress notes
  • Coordinate with medical and office staff for case consultations
  • Maintain professional licenses and CEU’s
  • Follow NASW code of ethics
  • Conducts individual and group counseling as appropriate
  • Refer patients to appropriate community resources and advocate for and assist the patient in obtaining those services
  • Provide crisis intervention and patient and family counseling as required
  • Report instances of neglect and abuse to appropriate authorities
  • Prepare complete and accurate case notes, write correspondence, reports, and other written materials
  • Document all aspects of service delivery in the permanent patient record in a timely manner
  • Participate in data collection and research activities related to the system and patient level outcomes
  • Participate in community-based activities as needed
  • Identify/report quality and risk management issues
  • Assist in development/revision and implementation of resource management policies and procedures
  • Participate in staff meetings, self-directed work teams, and other program initiatives related to performance improvement
  • Assume responsibility for professional self-development
  • Assist in facility-wide special projects and reports as deemed necessary
  • Other tasks as assigned to ensure the continued functioning of the organization
