Patient Care Coordinator $25/hour

*This is a completely back-office role – NOT patient facing*

Pay: $22-25/hour

Schedule: 40-hours per week, must be flexible to work 8-hour shifts during the hours of operation, must work 1 late shift per week

Monday 7AM-7PM
Tuesday 7AM-7PM
Wednesday 7AM-7PM
Thursday 7AM-5PM
Friday 7AM-5PM

Requirements: 

  • High school diploma or equivalent
  • 3+ years of Patient Care Coordinator / related experience
  • EMR experience (practice uses eClinicalWorks, NaviNet, Helio – training provided)
  • Orthopedic/Pain Management experience highly preferred

Responsibilities:

  • Triage and manage calls from patients/pharmacies/insurance companies and other facilities
  • Schedule patient appointments
  • Complete online insurance verification
  • Follow up with patients regarding missed appointments
  • Ensure patients have completed prescribed testing (imaging, labs, etc.) and have follow-up appointments schedule with their physicians
  • Ensure all documents are properly completed by their physicians
  • Prepare forms as requested by patients such as disability, MVA, etc.
  • Handle all attorney requests for their legal documents
  • Manage authorizations for motor vehicle and workers comp patients
  • Monitor webchat via Snap Engage daily and respond to appropriate parties
  • Manage calls from hospitals and physician offices and route calls to appropriate parties
  • Handle and coordinate the doctors’ daily schedules, including their calendars
  • Create electronic medical notes including general notes and telephone encounters as necessary
  • Inform patients about the practice CDC guidelines and requirements
  • Interact with other departments within the organization
  • Work as a liaison between patients and providers
  • Perform other duties as requested by management
