Clinical Case Manager / Educational Liaison – Youth Residential Care

Full-time, Monday to Friday, Exempt
Salary: $53,000 annually

Position Overview:
This role coordinates clinical case management and educational advocacy for youth in residential services. The Clinical Case Manager/Educational Liaison works closely with treatment teams to ensure comprehensive care planning and supports educational needs by acting as an advocate and liaison within school environments.

Key Responsibilities:

  • Schedule and coordinate all meetings related to treatment planning, including Individual Treatment Planning (ITP), team, and IEP meetings.

  • Serve as the educational liaison to support youth learning, ensuring IEPs meet their needs and advocating for youth in educational settings.

  • Maintain communication with families, workers, schools, and team members, including participation in milieu and staff meetings.

  • Collaborate with program clinicians on treatment plan development and coordination of planning meetings.

  • Assist in facilitating psycho-educational, parent, peer support, and independent living skills groups.

  • Coordinate follow-up care with child welfare and community partners.

  • Manage a caseload of individual and/or family clients as assigned.

  • Conduct intake reviews, interviews, and pre-admission evaluations.

  • Provide risk assessments, emergency safety planning, and thorough documentation of clinical work.

  • Attend required trainings, meetings, and maintain necessary certifications (e.g., CPR, First Aid, MAP).

  • Provide transportation for clients using agency or personal vehicles (with appropriate insurance coverage).

  • Offer after-hours on-call support as needed.

  • Continue professional development and perform other duties as assigned.

Qualifications:

  • Master’s degree in a clinical field.

  • Strong ability to analyze clinical and educational documents and interpret client information.

  • Problem-solving skills and ability to work with diverse populations.

  • Strong interpersonal and relationship-building skills.

  • Cultural competency is essential; bilingual skills or specialized clinical competencies are a plus.

  • Must have reliable personal vehicle and sufficient auto insurance coverage meeting specified minimums.
