Family Nurse Practitioner (FNP) – Outpatient Primary Care

  • Location: Jackson Heights NY
  • Schedule: Monday–Friday, 9:00am–6:00pm (30-minute unpaid lunch)
  • Compensation: $135,000–$160,000 annually (based on experience)

An established community health organization in Manhattan is seeking a Family Nurse Practitioner (FNP) to join their outpatient primary care team. This role focuses on providing comprehensive, culturally competent care to diverse patient populations, including women’s health, the LGBTQIA+ community, and individuals living with or affected by HIV/AIDS. The ideal candidate is passionate about health equity, has experience working with underserved or vulnerable populations, and thrives in a collaborative, mission-driven environment. Please note: PA-C candidates will not be considered for this position.

Responsibilities include:

  • Provide comprehensive primary care to a panel of adult patients
  • Evaluate and manage acute and chronic medical conditions
  • Perform initial assessment and management of mental health conditions (e.g., depression, anxiety)
  • Order, interpret, and act on diagnostic tests using evidence-based practices
  • Prescribe appropriate treatments and medications within the NP scope of practice
  • Counsel patients on treatment plans, preventive screenings, and lifestyle modifications
  • Refer patients to specialists as needed
  • Maintain accurate and timely documentation within the EMR system
  • Collaborate with a multidisciplinary care team to ensure coordinated patient care
  • Participate in clinic, provider, and team meetings
  • Contribute to quality improvement and peer review initiatives
  • Engage in health promotion and community outreach activities
  • Participate in shared night and weekend call coverage

Qualifications:

  • Master’s Degree (MSN) or Doctoral Degree (DNP) in Nursing
  • Completion of an accredited Nurse Practitioner program
  • Certification as a Family Nurse Practitioner or Adult Nurse Practitioner
  • Current and valid NYS NP license and DEA certification
  • Minimum of 2 years’ hands-on experience as an NP
  • Current BLS and infection control certification
  • Excellent bedside manner and communication skills
  • Proficiency in Spanish or an Asian language preferred
  • AAHIVM and/or LGBT health certification a plus
