RN Case Manager

Will be seeing patients in their homes, nursing homes, and assisted living facilities throughout Greater Boston

Pay: $50-55/hour + mileage reimbursement

Schedule: Part-time, day shift, exact hours are flexible and based on patients/their needs

  • Expected to see 5-6 patients per day
  • Have a dedicated NP who handles on-call coverage and nights/weekends – may occasionally need to cover if the NP takes a vacation or is sick

About the Role:

  • Providing hospice care to patients in their homes, nursing homes, and assisted living facilities
  • RN Case Manager will see 5-6 patients per day
  • Bi-weekly mandatory meetings at the office on Wednesdays after patients have been seen
  • Mileage reimbursement is provided

Requirements: 

  • Minimum 1-2 years of RN experience
  • Hospice and/or palliative care experience is required  
  • Graduate of an accredited nursing program
  • Active Massachusetts RN License
  • BLS Certification

Responsibilities:

  • Perform comprehensive initial and ongoing patient assessments, including physical, psychosocial, emotional, and spiritual needs
  • Develop, implement, and update individualized care plans in collaboration with physicians and the interdisciplinary team
  • Provide skilled nursing care focused on comfort, pain management, and symptom control
  • Administer medications and treatments as prescribed, monitoring for effectiveness and side effects
  • Educate patients and families regarding disease progression, symptom management, medication administration, and end-of-life expectations
  • Serve as the primary case manager, coordinating care among physicians, social workers, chaplains, home health aides, and other team members
  • Provide emotional support and guidance to patients and families during end-of-life process
  • Respond promptly to changes in patient condition and communicate updates to the physician and care team
  • Maintain accurate, timely, and compliant clinical documentation
  • Ensure adherence to hospice regulations, policies, and standards of care
  • Assist families with preparation for death and provide post-mortem care in accordance with agency policy
  • Support caregivers by offering training, resources, and practical guidance
