Will be seeing patients in their homes, nursing homes, and assisted living facilities throughout Greater Boston

Pay: $50-55/hour + mileage reimbursement

Schedule: Part-time, day shift, exact hours are flexible and based on patients/their needs

Expected to see 5-6 patients per day
Have a dedicated NP who handles on-call coverage and nights/weekends – may occasionally need to cover if the NP takes a vacation or is sick

About the Role:

Providing hospice care to patients in their homes, nursing homes, and assisted living facilities
RN Case Manager will see 5-6 patients per day
Bi-weekly mandatory meetings at the office on Wednesdays after patients have been seen
Mileage reimbursement is provided

Responsibilities:

Perform comprehensive initial and ongoing patient assessments, including physical, psychosocial, emotional, and spiritual needs
Develop, implement, and update individualized care plans in collaboration with physicians and the interdisciplinary team
Provide skilled nursing care focused on comfort, pain management, and symptom control
Administer medications and treatments as prescribed, monitoring for effectiveness and side effects
Educate patients and families regarding disease progression, symptom management, medication administration, and end-of-life expectations
Serve as the primary case manager, coordinating care among physicians, social workers, chaplains, home health aides, and other team members
Provide emotional support and guidance to patients and families during end-of-life process
Respond promptly to changes in patient condition and communicate updates to the physician and care team
Maintain accurate, timely, and compliant clinical documentation
Ensure adherence to hospice regulations, policies, and standards of care
Assist families with preparation for death and provide post-mortem care in accordance with agency policy
Support caregivers by offering training, resources, and practical guidance
