Outreach Worke-Case Manager Mon-Fri 8-5pm $22hr
Position Summary
This position will perform direct outreach to unsheltered and sheltered homeless populations by informing,
encouraging and assisting them in accessing appropriate programs, resources, services, and housing. Intensively
work with individuals experiencing homelessness to facilitate linkages to a range of services and resources.
Safely operate the Street Outreach Shower Van while following all SHC policies and procedures. Ensure all
clients are compliant with program rules, individual service plans and lease requirements. Input all unsheltered
clients and engagements into the Homelessness Management Integration Software (HMIS)
Essential Functions, Duties and Responsibilities
Outreach
• Regularly conduct outreach in areas known to have a high concentration of homeless individuals,
including parks, encampments, and other public spaces.
• Establish trust with individuals experiencing homelessness through consistent engagement, empathy,
and non-judgmental support.
• Assess clients’ needs and vulnerabilities, including health, safety, substance use, and housing, and
develop individualized plans to address these needs.
• Provide referrals to shelters, food resources, medical services, substance use treatment, mental health
support, and other relevant community services.
• Attend individual supervision, staff and community meetings; and
• Respond to crises, including mental health emergencies, substance overdoses, and safety concerns, by
providing immediate support and intervention.
• Maintain accurate and timely documentation of outreach activities, client interactions, and service
referrals in accordance with organizational policies.
• Work closely with local government agencies, shelters, hospitals, and law enforcement to coordinate
services and address issues related to homelessness in the community.
• Ensure consistent follow-up with clients to monitor progress, provide ongoing support, and encourage
continued engagement with services.
