Case Manager

- Full time – Temp to Hire
- Monday – Friday, 8:00 am – 5:00 pm
Job Requirements:
- Perform direct outreach to unsheltered and sheltered homeless populations by informing, encouraging and assisting them in accessing appropriate programs, resources, services, and housing.
- Regularly conduct outreach in areas known to have a high concentration of homeless individuals, including parks, encampments, and other public spaces.
- Establish trust with individuals experiencing homelessness through consistent engagement, empathy, and non-judgmental support.
- Assess clients’ needs and vulnerabilities, including health, safety, substance use, and housing, and develop individualized plans to address these needs.
- Provide referrals to shelters, food resources, medical services, substance use treatment, mental health support, and other relevant community services.
- Attend individual supervision, staff and community meetings.
- Respond to crises, including mental health emergencies, substance overdoses, and safety concerns, by providing immediate support and intervention.
Pay Rate:
- $22/hour

You will be entitled to health insurance once employed for 60 days and you must work 30 hours per week, on average, per month to enroll and maintain health insurance through Complete Staffing Solutions.
