HomeBase Case Manager (Bilingual Spanish)
Pay: $25–$27/hour
Schedule: Mon–Fri, 8:00 AM–5:00 PM (40 hrs/week)
Location: Onsite & field-based (mileage reimbursed)
Overview
The Case Manager supports individuals and families experiencing homelessness by providing re-housing and stabilization case management. This role focuses on client assessments, housing search assistance, service coordination, and ongoing client support to promote housing stability and self-sufficiency.
Key Responsibilities

Conduct client intakes, assessments, and re-housing plans

Provide case management and housing search support

Assist clients with subsidies, documentation, and move-in resources

Coordinate referrals to social, employment, medical, and community services

Maintain accurate client records and reports in web-based systems

Manage an assigned caseload and provide excellent customer service
Qualifications

Bilingual English/Spanish required

1–3 years of human services or case management experience preferred

Strong communication, organization, and customer service skills

Comfortable working in a fast-paced, client-facing environment

Proficient with Microsoft Office and database systems
Education & Requirements

Bachelor’s degree preferred, not required

Valid driver’s license and reliable transportation required

Must obtain R.E.B. certification within 6 months
