Outpatient Family Practice – Manchester, CT
Salary: $118,000 – $125,000 + $2,500 bonus for bilingual Spanish speakers

A community-focused health center is seeking a full-time Family Nurse Practitioner (FNP) or Physician Assistant (PA) to join their outpatient team in Manchester, Connecticut. This is a high-impact opportunity with a well-established organization focused on reducing barriers to care and improving community health outcomes.

The team is currently navigating multiple staff vacancies, so your contribution would be immediately valued. All providers are asked to commit to a 24-month agreement.

Schedule:

  • Monday–Wednesday: 8:00 AM – 4:30 PM

  • Thursday: 8:00 AM – 6:00 PM

  • Friday: 8:00 AM – 3:00 PM

  • Saturday: 8:00 AM – 2:00 PM (as needed)

  • Coverage includes two Manchester locations

Requirements:

  • Must have at least 1 year of relevant experience (2+ preferred)

  • Graduate of an accredited FNP or PA program

  • Active RN (if FNP), CT state license, CT controlled substance license, and DEA

  • BLS Certification

  • Experience in outpatient primary care

  • Bilingual (Spanish) strongly preferred

Key Responsibilities:

  • Evaluate and assess patient conditions

  • Conduct routine and specialized physical exams

  • Develop care plans and prescribe medications

  • Order and interpret diagnostic tests

  • Provide health counseling and patient education

  • Coordinate follow-up care and specialist referrals

  • Maintain accurate EMR documentation

  • Collaborate with a multidisciplinary team
