Program Coordinator

Schedule: Monday through Friday 8:00AM-4:00PM / 9:00AM-5:00PM (will vary by day)
No weekends, no on-call

Requirements: 

  • BSN highly preferred, will consider ASN
  • New Jersey RN License
  • 3-5 years of nursing experience, 2 years of nursing experience in OB/GYN, Internal Medicine, Family Medicine, Pediatrics
  • CPR and AED Certificates
  • Must maintain professional development requirements per NJ Division of Consumer Affairs, Board of Nursing
  • Experience with eClinicalWorks is helpful

Responsibilities:
Mix of clinical and administrative duties –

  • Serve as Obstetrics Program Coordinator under the Sr. VP of Nursing and Clinical Operations
  • Provide oversight and management of program operations, with the purpose of improving pediatric health outcomes by piloting the addition of quality prenatal primary care services
  • Coordinate the clinical and operational infrastructure necessary to incorporate prenatal care into community health center
  • Work with key administrative staff in various departments across various sites as well as with critical support staff, participate in early and comprehensive prenatal care, post-partum, and pediatric care and identification of a medical home
  • Guide the process of identification of new pregnant and high-risk patients; coordinate essential linkages to nutrition services, behavioral health, and oral health care as well as to adjunct supports including community health workers and health education programming
  • Serve as a team lead in the cultivation of relationships and institution of viable processes with partnering hospital and other community-based organizations toward provision of comprehensive services, elimination of service gaps, avoidance of service duplication, and reduction of significant access barriers to care for at-risk mothers and their babies
  • Implement and oversee an integrated comprehensive maternal and child health program
  • Work in collaboration with other departments including oral health, behavioral health, nutrition, and special populations
  • Implement policies and procedures for registration process, eligibility determination, workflows, case management, and referrals as necessary to address various factors influencing pregnancy outcomes and pediatric outcomes, including chronic disease management, healthy weight and nutrition, oral health care, domestic violence, and substance abuse
  • Implement staff training including education on topics including prenatal counseling, multi-vitamins and folic acid supplementation, breast feeding, and interval birth spacing
  • Identify and cultivate relationships with key at affiliated hospital in order to facilitate bidirectional referrals
  • Implement processes for managing integration of health records and health data
  • Establish provider-to-provider electronic communications and update the forms, policies, and procedures to execute a viable, sustainable prenatal program in concert with affiliated hospital
  • Identify and cultivate formal linkages and informal partnerships with community agencies and entities providing direct or related services to maternal and child healthcare
  • In conjunction with Sr. VP of Dental Operations, coordinate the oral health education to raise awareness of the benefits of oral health care during pregnancy and on overall and reproductive health, as well as the benefits of oral healthcare and the establishment of a dental home for infants and babies prior to their first birthday
  • Ensure case-management and patient navigation for integrating linkages between oral and medical health services
  • Oversee the implementation of program requirements including the accurate completion of Presumptive Eligibility for pregnant patients, and involvement in the Continuous Quality Improvement process
  • Work alongside patient care coordinator, women’s health, and pediatric areas to facilitate smooth and efficient patient flow
  • Oversee case-management and referral processes
  • Participate in the development of health literate brochures and web-based materials to outreach and educate target population and oversee the dissemination of all program-related resource literature
  • Maintain over-all responsibility for all documentation requirements including timely data entry and analytical reporting as required by NJ State Department of Health, HRSA, community health administration, and/or other relevant credentialing entities
  • Prepare and submit narrative progress reports and statistical summaries
  • Provide completed expenditure reports
  • Collaborate with CMO, Sr. VP of Dental Services, and others within the community health center to facilitate seamless integration of program goals, objectives, and methodology into the service delivery system, including IT system and electronic health record changes
  • Follow Healthy Start guidelines, established protocols, and procedures for the care of prenatal and postpartum patients, including regular assessments, acute evaluations, referrals to providers, and ensure proper documentation of all interventions
  • Attend trainings, meetings, and conferences related to patient care and other related services including Infection Control/OSHA, Patient Safety, Risk Management, and QA/PI programs
  • Participate with collaboratives and other improvement processes related to clinical patient services
  • Learn and apply the trauma informed care principles within the scope of the position
