Salary: Up to $140,000
Schedule: Monday through Friday 8AM-4PM or 9AM-5PM, onsite
Requirements:
- Active NYS Psychiatric Nurse Practitioner license
- Board eligible or certified as Psychiatric Nurse Practitioner
- Ideal candidate needs to be an NYS Medicaid provider
Responsibilities:
- Reports to Director of Behavioral Health and Chief Medical Officer
- Performs psychiatric evaluations and assessments for patients to ascertain their diagnoses and establish treatment plans
- Collaborate with Psychiatrist pertaining to documentation and clinical decisions
- Provides SBIRT encounters as part of the Substance Abuse Expansion and Integration Program
- Provides medication management including education/review of side effects and treatment options with patients so they may give informed consent
- Provides individual psychotherapy
- Provides crisis intervention and/or counseling to diffuse conflicts among patients
- Completes medical forms and documentation related to patient care including any necessary to facilitate transfer, and if indicated, referrals to outside programs or psychiatric clinics
- Makes and/or tracks all referrals to other mental health agencies and providers
- Consults with primary care providers to focus on psychiatric and substance abuse issues
- Works collaboratively with specialists for consultation and referral purposes
- Coordinates patient’s psychiatric care with medical clinic and outside providers
- Provide coverage for Psychiatrist regarding supervision of clinical staff when further guidance in patient care may be needed
- Coordinates emergency responses, including a team to minimize patient’s danger to self and others, direct discussion with EMS/FDNY regarding patient’s condition, referral to psychiatric or medical emergency departments, and follow-up while patient is in the emergency department or upon release
- Maintains ultimate responsibility for directing the course of the patient’s psychiatric care and treatment plan within the scope of their expertise while collaborating with Psychiatrist
- Encourages health promotion, disease prevention, and health maintenance
- Provides patient advocacy, as appropriate
- Participates in case-conference and case management activities
- Meets with mental health team members on a weekly basis to discuss/implement recommendations and collaborate toward delivering quality psychiatric services to patients
- Collaborates with the Registration and Scheduling Departments to ensure appropriate patient scheduling and flow
- Collaborates with the HIM Department to ensure appropriate documentation procedures
- Observes, implements, and promotes the federal regulations as defined by HIPAA and Policies and Procedures of the company
- Makes suggestions to the Director of Behavioral health and the Chief Medical Officer to improve patient care
