Hiring for a Registered Dietician – Diabetes Educator for a clinic in Lawrence, MA

Schedule: Monday through Friday 8am-4:30pm OR 8:30am-5pm
Pay: $40/hr

Position Summary:
Participates directly in patient care through education for health promotion and disease prevention and assists patients manage and understand chronic illness. Utilizes skills and knowledge to ensure patient understanding and to facilitate patient compliance and optimal patient health provided to clients and their families. Demonstrates an understanding of the concept of integrated care teams (ICT) and has an understanding of team goals.

Job Responsibilities and Performance Standards:

  • Provides individualized diabetes self-management education/training to individuals and groups according to the American Diabetes Association Education Recognition Program standards and scope of practice.
  • Assist patients with the development of short and long term goals, engaging and motivating patients to reach optimal health.
  • Assesses patient’s diabetes self-care knowledge, practices, skill level and learning needs.
  • Assesses patient’s barriers to care and learning.
  • Provides diabetes self-management education in the context of the patient’s cultural, socio-economic, psychosocial and health literacy needs.
  • Assesses patient’s level of diabetes control and consults with health care team to improve health care indicators and patient’s quality of life.
  • Participates in integrative care teams, coordination of care and quality improvement efforts.
  • Reports for scheduled shifts on time and in professional manner with regard to behavior, telephone courtesy, dress, identification, and communication with patients, colleagues, and outside agencies. Adheres to organization’s attendance policy.
  • Assessment: Assess and documents physical and psycho-social problems of individuals and their families.
  • Performs basic intake using appropriate form as relevant on clients seeking care; record history that may include medical, family, substance use, and present living arrangements.
  • Identifies need for client/family teaching and or/referrals.
  • Prioritizes client’s needs based on assessment and client’s verbalized areas of concern.
  • Participates in a multi-disciplinary approach directed toward client’s needs and care. Consults with other health care providers (i.e. social workers, MD, nutritionist, N.P., etc.) when appropriate.
  • Implementation: Implements care based on assessment data, approved protocols, sound knowledge base and knowledge of available referral network, as evidenced by documentation on client’s records.
  • Evaluation: Evaluates care provided and client’s response to recommended course of treatment.
  • Accurately documents changing needs and client’s response to course of treatment.
  • Explains procedures to patients, significant other, or other family member in a clear, concise manner, with sensitivity to clients’ level of literacy, culture, and language, learning preferences and learning barriers.
  • Documents all teaching information and materials given to the patients, or family members including their response and need for further follow-up.
  • Conducts classes in order to educate groups of clients in issues relevant to health care needs of specifically designated populations.
  • Obtains 15 CEUs per calendar year in diabetes-related topics.
  • Maintains and submits data from patient encounters according to company and special program requirements.
  • Participates in the orientation/teaching process, when requested, to assure that new employee/student will become familiar with policies, procedures, and expectations for the job role.
  • Participates in community education and promotion regarding patient education when requested, as time available.
  • Specific Care Categories: Provides specific care in the areas as related to patients’ unique physical and psychological needs.

Qualifications:
Experience

  • Registered Dietitian–MA license
  • Minimum of 1-3 years as a clinical dietitian including experience as a diabetes educator in an ambulatory, community health setting.
  • Certified Diabetes Education Specialist (CDES) preferred (if not currently a CDES, candidate is required to sit for exam within 6-12 months of hired.
  • Bilingual (Spanish) preferred

Education

  • Graduate of an accredited institution
  • Current Massachusetts Registered Dietitian License
  • Current BLS certification
