Hiring for a RN Navigator Neurology role with a healthcare facility in Worcester, MA.

One year contract – possible extensions/permanent conversion
Shift: M-F 8:00 am – 4:30 pm
Department: STROKE BUNDLE
Pay rate: $58/hr
1-year assignment. BSN and 2-years of experience with Neurology required.
This position entails following a patient that has had a stroke or seizure 90 days post discharge. Follow up with the patient if they go to a SNF/LTAC, make specialist or PCP follow up appointments, medication management, home healthcare and will assist with getting the patient to a clinic if there are noted concerns. homecare or case management experience preferred.
