Care Delivery Manager (RN) — Ridgewood Queens
Salary: $120K–$125K | Schedule: M–F, 8:30 AM–5 PM | Weekly Hours: 37.5 | Setting: In-person clinic + some field work

Key responsibilities:

  • Own personalized care plan execution that improves patient satisfaction and outcomes.

  • Lead and facilitate Interdisciplinary Team (IDT) meetings, including virtual collaboration and documentation management.

  • Serve as after-hours manager on call—handle urgent follow-ups, grievances, and incident investigations.

  • Oversee staffing, resource allocation, and workflow to guarantee seamless clinical operations.

  • Collaborate with clinicians on assessment tools and quality metrics (UAS, documentation, compliance).

  • Manage care transitions with clinical teams to optimize patient outcomes across care settings.

  • Drive regulatory compliance across all documentation and clinical procedures in alignment with CLHC and federal/state standards.

  • Lead HR functions for direct reports: hiring, onboarding, coaching, performance evaluations, and disciplinary actions.

  • Act as Director of Patient Services (DPS) as assigned.

  • Maintain expertise on current clinical best practices, health issues, and emerging trends.


Must-have qualifications:

  • RN license, active and unrestricted in NY State (mandatory).

  • Completed accredited nursing program (BSN preferred; minimum 5 years clinical experience).

  • Minimum 2 years in a management role within home health, long-term care, or acute care settings.

  • Proven experience supervising staff and managing clinical operations.

  • Skilled in working with frail/elderly populations or willingness to receive targeted training.

  • Strong communication, problem-solving, and leadership skills.
