Social Worker ll (MSW)
Location:Passaic, NJ

Shift: M-F 8am- 4:30pm
1 Saturday a month
Interview: on-site
Pay rate:
$45-$50/hr

Requirements:
- Master’s degree in Social Work OR in Marriage and Family Therapy or related field
- Minimum 1 year hospital experience
- NJ Social Worker license
- BLS/CPR a plus, not required
- EMR experience a plus (they use EPIC)
- Discharge experience a must
Responsibilities:
- Provide emotional support, crisis intervention and counseling for Pediatrics, Adolescents, Adults, Older Adults and their significant others to cope with a multiplicity of social and emotional problems
- Referrals for financial assistance and supportive services to patients and families with problems that are related to hospitalization, illness, disability or trauma.
- Provide case management & milieu management
- Complete psychosocial assessments,
- Assist in the development and re-evaluation of treatment plans and work as a member of a multidisciplinary team
- Coordinate discharge planning.
- Complete necessary documentation and maintain clinical record for each client during his/her length of stay.
Onboarding/Credentialing:
- Background check (ADP, OIG, SAM, CORI)
- CSS Onboarding
- 10 panel drug screen
- Hep B
- MMR
- Tdap
- Varicella
- 2 Step PPD or Chest X-Ray if employee is a positive reactor
- Flu
- COVID vaccine and Booster OR Medical/Religious Exemption
